Needing more identification cards for your DNA Test kit to attach to and send more samples?  This is the download for those!
 
These are also acceptable for sending me samples from the individual blood DNA tests.  If you have multiple samples to send at a time simply download these identification cards, cut them apart and staple them to each zip lock baggie!
